Patient Corner

Here you will find the information you need before coming to our office.  We know that your time is valuable and doctors appointments are often squeezed into a busy schedule.  In order to reduce the time you spend in our office we encourage you to print out our intake forms and fill them out before your visit.

FORMS

Please select the type of form you need to download from the list below and print it out. 

If you will be using health insurance then you will print out 'Health Insurance Forms'.  If you will be paying out of pocket you will print out 'Self Pay Forms'...etc.

• Health Insurance Forms-print if using health insurance
• Self Pay Forms-print if paying out of pocket
• Workers Compensation Forms-print if using workers compensation
• Motor Vehicle Accident Forms-print if you were in a car accident

Please note that we do not accept 3rd party insurance, such as, the car insurance of another driver.  We can only accept your own personal injury protection (PIP) car insurance as a form of payment.  If the accident was the fault of another driver you can file a claim with your own car insurance, you can use your health insurance or you can pay out of pocket.  If you have questions regarding this policy please contact us at 512-478-1613.

  If you have not yet scheduled your appointment    please contact us.  Initial appointments last an hour to   an hour and a half and follow up appointments are about 15 minutes. Please bring your completed paperwork with you to your appointment.
